Parental Consent Forms

Parental consent form and minor liability waiver are both needed when the minor (under 18 years of age) is riding at FLWB Karting and their parent or legal guardian is not present at the time of booking.  Consent forms for non-members are good for 1 day only. Consent forms for members are good for one year.

If a parent or legal guardian is present, then we provide a different form that does not need notarized.

*Forms must be printed in COLOR and notarized by a
notary public to be valid.  (Don’t have a color printer?  Call us at

forms.)
