Both notarized forms are needed when the minor(under 18 years of age) is riding at FLWB Karting for the first time and their parent or legal guardian is not present.  Once this form is validated, it is good for one year and no additional consent form is needed.

If a parent or legal guardian is present, then we provide another form that does not need notarized.  Once this form is validated, it is good for one year and no additional consent form is needed.

Forms are kept on file and valid for one year.  Once the consent forms are presented the minor can enjoy karting for the next year without being accompanied by their parent.
